Application for Employment Certificate - Working Papers 2021-2022

To begin the process to obtain your NYS Working Papers:

The Health Office needs a copy of a current physical (dated within the year).

Your Parent/Guardian needs to complete Part 1 of the application for employment certificate. The form is available on the district website. Search for 'Working Papers.'

When the form and the physical are completed, it can be returned your building Health Office via email, fax or in person.
