Schedule NECSD Student Vaccination (Pfizer), 12+ - June 1, 2021
The Newburgh Enlarged City School District is planning to host a vaccine distribution next week. If interested, please use the link below to schedule an appointment.
If you are able to complete the consent forms prior to your appointment time, that will expedite the check-in process. Consent forms can be found here.
Scholars who are interested can share this form with family members if they would like to receive the vaccine together.

Tanto los padres como los estudiantes deben traer una identificación adecuada para recibir la vacuna. Traiga la información del seguro si la tiene (certificado de nacimiento, tarjeta de seguro social, identificación de los padres, licencia de conducir, pasaporte, matricula consular, identificación municipal, etc.). Si no tiene seguro, está bien, aún podrá vacunarse.
Los académicos que estén interesados pueden compartir este formulario con miembros de la familia si desean recibir la vacuna juntos.
