The Pre-Kindergarten Health Office
Minimum Immunization Requirements for Entrance into Pre-Kindergarten:
| Diptheria Toxoid Containing Vaccine (DTap, DTP) | 3 doses |
| Tetanus Toxoid Containing Vaccine and Pertussis Vaccine (DTaP, DTP) | 3 doses if born on or after 1/1/2005 |
| Polio (IPV or OPV) | 3 doses |
| Measles, Mumps and Rubella (MMR) | 1 dose (may be given no earlier than four days prior to the first birthday) |
| Hepatitis B | 3 doses |
| Haemophilus influenza type b (Hib) | 3 doses if less than 15 months of age OR 1 dose given on or after 15 months of age |
| Varicella (Chickenpox) | 1 dose (may be given no earlier than four days prior to the first birthday) |
Additional Documents Required for Pre-Kindergarten Attendance:
- Proof of Lead Screening
- Copy of Most Recent Physical Exam, Appointment Card for Physical Exam or Signed Permission for School Physical Exam
- Dental Screening Certificate if child was seen by a dentist
