Health Insurance Benefit Information - Open Enrollment
The District’s Open Enrollment period ends Wednesday, November 30, 2011. If you are electing to change your health insurance plan, all enrollment/cancellation forms and required documentation must be submitted to the Health Benefits Department on or before that date. All changes will go into effect on January 1, 2012. If the required forms are not submitted to the Health Benefits Department, your insurance plan will not be changed. If as an HMO subscriber you do not elect to change plans and you fall into the category where there is a differential payment due; it will automatically be deducted from your paycheck through your health insurance deduction effective the first pay date of January 2012. All health insurance changes or differential payments will continue until you elect to make a change during next year’s Open Enrollment period effective October 1, 2012-November 30, 2012.

Important 2012 Medicare Information
The Department of Health & Human Services has announced that the standard monthly Medicare Part B premium for a Medicare eligible individual in 2012 will be $99.90. For 2012, this will be the only standard rate of reimbursement and will replace the 3 standard rates for 2011 which are currently $96.40, $110.50 and $115.40.

October and November are Open Enrollment Months
October and November are open enrollment months during which employees may add health insurance or make changes to your current health insurance plan, for an effective date of January 1st. Such changes require a transaction form to be completed and returned to Keisha Martinez in the Health Benefits Department no later than November 30th of the given year. Open Enrollment
October 1st –November 30th, 2011 is the District’s Open Enrollment period for Health Insurance. Changes can be made to your health insurance coverage during this time. Please note: All changes will go into effect on January 1st, 2012.

Preferred Group Visit
Representatives from the Preferred Group will be visiting the District on Thursday, September 22, 2011 to provide information on the Flex Plan. Flex Plan information and a limited supply of enrollment applications for the 2011-2012 school year will be sent to each building shortly. Forms can also be found on-line on the District’s website under the Health Benefits or Payroll Department web page. All enrollment forms for the 2011-2012 school year must be submitted to the Payroll Department by September 30, 2011. Below is the Preferred Group schedule for the September 22, 2011 visit:

IMPORTANT INFORMATION FROM THE NYSTRS
The NYSTRS has asked that we publicize for both active members and retirees the existence of unclaimed and abandoned funds held by NYSTRS. You can find out more information by visiting http://www.nystrs.org. Select the Unclaimed Funds tab at the bottom of the home page, visitors will be directed to two lists: Unclaimed Accounts and Abandoned Accounts. Both lists are alphabetical by last name and display last known teaching location or place of death. The Abandoned Accounts list, which contains significantly more entries, also contains a search feature using either last name or last known teaching location.

2011-2012 Health Insurance Buy-Out Information
The 2011-2012 Health Insurance Buy-Out Forms will be available in the Main office of each school on May 2, 2011. You are only required to complete the form if this is your first time participating in the Health Insurance Buy-Out. If you are interested in participating, please complete the Buy-Out form and attach a copy of your current health insurance card and submit to the Health Benefits Department by June 1st, 2011. Employees who already participate in the Health Insurance Buy-Out will be automatically enrolled for the 2011-2012 school year. If you are currently participating in the Buy-Out and wish to re enter the District’s health insurance plan effective September 1, 2011, please submit your request in writing to the Health Benefits Department by June 1st, 2011. You will also need to complete an enrollment form and submit documentation if you are electing to participate in family coverage. Most enrollment forms can be found on the Health Benefits Department Page located on the District’s web site. Please be advised that all forms and requests to re enter the District’s health insurance plan, must be received by the deadline of June 1st, 2011. There will be no exceptions.

NYS Retirement Seminar Information:
For those employees who received correspondence and want to attend the NYS and Local Retirement Seminar:

Please be advised that the New York State and Local Retirement System Seminar will be held on the following date and time:

Time: 5:00pm-7:00pm
Date: Wednesday, April 13, 2011
Place: Board of Education Auditorium

Please note that the seminar will begin promptly at 5:00pm. It is suggested that you bring paper and pen and/or pencil to take notes. If you previously submitted correspondence that you were interested in the seminar and now you cannot attend, please contact the Health Benefits Department at ext.33467. by March 18, 2011, so we will have an accurate total number of participants. If we do not hear back from you, your name will be added to the list of seminar participants.
