Important Information for MVP PPO Enrollees:
Medical:
There seems to be some confusion as to how the debit card system should work. In order to clarify please be advised that you should not pay your medical provider at the time of service. You should only show your MVP ID card when you go to your participating physicianís office. The physicianís office will bill the insurance company and you should also receive a copy of that bill from your medical provider. You will receive an explanation of benefits (EOB) from the insurance company which will detail your payment responsibility to the provider. You will then at that time contact your providerís office to pay the amount due based on the EOB by giving the physicianís office your debit card information. The amount owed to the doctor is based on the EOB and will be deducted from the debit card and the deductible.

2013-2014 HEALTH INSURANCE BUY-OUT INFORMATION
Everyone who participates in the Health Insurance Buy-out for the 2013-2014 school year must complete a Health Insurance Buy-Out Form and submit it to the Health Benefits Department by Friday, June 14, 2013. You must also attach a copy of your current health insurance card. Forms will be available in your buildingís main office next week.

New Flex Plan Information for 2012
Important Information: The new Flex Plan year begins on October 1st, 2012. The Districtís new Flex Plan carrier will be Aflac. Those who were participants in the Flex Plan under the Preferred Group will still have until December 31, 2012 to submit Flex claims incurred prior to September 30, 2012, directly to the Preferred Group.

The Open Enrollment period for the Districtís Flex Plan will be during the month of September. Enrollment forms (SRA) and plan information will be available under both the Payroll and Health Benefits pages of the Districtís web site. Aflac Representatives will be visiting each building during the Open Enrollment period and will be accepting enrollment forms on site for the Aflac Flex plan and Aflac Supplemental Insurance Plans ie (Cancer Indemnity, Disability and Accident Indemnity). You can also submit the Flex Plan enrollment form (SRA) directly to the Payroll Department. An Aflac Representative schedule is forthcoming and will be posted on our website and in your building. The last date to submit all enrollment forms during the open enrollment period will be September 25, 2012.
  • Please note you will not receive an enrollment packet. All plan information is available on line and will also be available with the Representatives on the dates of their visits.
  • Please use 19 as the number of pay periods when calculating the deduction per pay for the Flex Plan.

Health Insurance Buy-Out 2012-2013
On May 1, 2012, Health Insurance Buy-Out forms will be available in the main office of each building. If you are interested in enrolling in the Health Insurance Buy-Out for the first time, please complete the Buy-Out form and attach a copy of your current health insurance card. Forms must be submitted to the Health Benefits Department by Friday, June 1, 2012. If you are currently participating in the Health Insurance Buy-Out; you do not need to complete another form. You will automatically be enrolled in the Buy-Out for the 2012-2013 school year. If you wish to opt out of the Health Insurance Buy-Out and re enroll in the Districtís Health insurance plan for an effective date of September 1, 2012; you must put your request in writing and submit it to the Health Benefits Department by Friday, June 1, 2012. Please be advised that Buy-Out forms or opt out letters will not be accepted after June 1, 2012.

Urgent Message : March 31, 2012 Deadline to Enroll in Tier 5 Membership
Tier 6 Membership in the NYS and Local Retirement System and the NYS Teachersí Retirement System will be in effect as of April 1, 2012. Employees who are not registered with a retirement system, but are eligible to do so through their employment will have until midnight March 31, 2012, to fulfill the requirements to lock into Tier 5.

The requirements include; filing the necessary enrollment information through the District and working at least one day in a system-eligible job before the end of March. This applies primarily to substitutes and part-time employees

Health Insurance Benefit Information - Open Enrollment
The Districtís Open Enrollment period ends Wednesday, November 30, 2011. If you are electing to change your health insurance plan, all enrollment/cancellation forms and required documentation must be submitted to the Health Benefits Department on or before that date. All changes will go into effect on January 1, 2012. If the required forms are not submitted to the Health Benefits Department, your insurance plan will not be changed. If as an HMO subscriber you do not elect to change plans and you fall into the category where there is a differential payment due; it will automatically be deducted from your paycheck through your health insurance deduction effective the first pay date of January 2012. All health insurance changes or differential payments will continue until you elect to make a change during next yearís Open Enrollment period effective October 1, 2012-November 30, 2012.

Important 2012 Medicare Information
The Department of Health & Human Services has announced that the standard monthly Medicare Part B premium for a Medicare eligible individual in 2012 will be $99.90. For 2012, this will be the only standard rate of reimbursement and will replace the 3 standard rates for 2011 which are currently $96.40, $110.50 and $115.40.

The NYSTRS has asked that we publicize for both active members and retirees the existence of unclaimed and abandoned funds held by NYSTRS. You can find out more information by visiting http://www.nystrs.org. Select the Unclaimed Funds tab at the bottom of the home page, visitors will be directed to two lists: Unclaimed Accounts and Abandoned Accounts. Both lists are alphabetical by last name and display last known teaching location or place of death. The Abandoned Accounts list, which contains significantly more entries, also contains a search feature using either last name or last known teaching location.

2011-2012 Health Insurance Buy-Out Information
The 2011-2012 Health Insurance Buy-Out Forms will be available in the Main office of each school on May 2, 2011. You are only required to complete the form if this is your first time participating in the Health Insurance Buy-Out. If you are interested in participating, please complete the Buy-Out form and attach a copy of your current health insurance card and submit to the Health Benefits Department by June 1st, 2011. Employees who already participate in the Health Insurance Buy-Out will be automatically enrolled for the 2011-2012 school year. If you are currently participating in the Buy-Out and wish to re enter the Districtís health insurance plan effective September 1, 2011, please submit your request in writing to the Health Benefits Department by June 1st, 2011. You will also need to complete an enrollment form and submit documentation if you are electing to participate in family coverage. Most enrollment forms can be found on the Health Benefits Department Page located on the Districtís web site. Please be advised that all forms and requests to re enter the Districtís health insurance plan, must be received by the deadline of June 1st, 2011. There will be no exceptions.

Please be advised that the New York State and Local Retirement System Seminar will be held on the following date and time:

Please note that the seminar will begin promptly at 5:00pm. It is suggested that you bring paper and pen and/or pencil to take notes. If you previously submitted correspondence that you were interested in the seminar and now you cannot attend, please contact the Health Benefits Department at ext.33467. by March 18, 2011, so we will have an accurate total number of participants. If we do not hear back from you, your name will be added to the list of seminar participants.
